| Cleavage Determination in Breast Augmentation: | ||
| On the previous page, cleavage determinants relating to the native chestwall shape were discussed. On this page the impact of the presence, or absence, of fatty soft tissues, and | position of the nipples, and breast mounds, relative to the sternal midline, are considered. | |
Thin Chestwall Tissues with Little Fat:
| A 37 year old with very thin tissues is shown before and after trans-ax augmentation with smooth saline implants. | ![]() |
![]() |
Notice the breast shape and natural slope, despite very thin tissues. The lack of fatty tissues in the transition from breasts to sternum causes the space between the breasts to appear a bit wide. Vertically oversized pockets allow for the natural shape, and help to prevent rippling in this patient. |

The implant shape is more apparent due to the lack of soft tissues of the breast and chest. Fatty tissues hide the outline of the implants, and soften the look of the result in breast augmentation. Still no ripples are seen in this patient as a result of the oversized pockets which allow a natural shape of the breasts. |
Widely Positioned Breasts
| A 25 year old with widely spaced breasts is seen before and after trans-axillary breast augmentation. | ![]() |
![]() |
Note the natural shape and slope of the breasts, but a moderately wide separation between the breasts due to the broad spacing of the breasts pre-op. The implants must be centered behind the nipples. If the implants were to lie closer, then the nipples would lie too far to the sides on the breast mounds. |

Natural slope and breast mound shapes are aided by development of the vertically oversized pockets. |
