Excess skin and fatty tissues beneath the jaw lead to a double chin deformity. Many younger patients with this problem can be treated by Tumescent Liposuction of the excess fat or by direct fat excision via a small incision under the chin. This approach allows fat removal from the neck and jowl region and, via the incision under the chin, direct tightening of the neck's superficial platysma muscles to establish a better angle between the neck and jawline. Sometimes a chin implant is added to better balance chin projection and correct under-projecting chin or hypogenia. If there is too much loose neck skin and muscle, a Facelift / Necklift procedure is necessary to correct the age changes. Liposuction of the neck region and direct fat excision with platysma muscle tightening under local anesthesia , as well as a procedure that added a chin implant in my AAAASF-certified OR is demonstrated below. |

| Case 1: A 33 year old with a small chin and fatty accumulation in the submental region (double chin) underwent tumescent liposuction of the submental area and jawline, along with direct vision removal of fat and tightening of the muscles below the chin and of of the anterior neck. A small chin implant was also inserted to correct the small chin and add apparent length to the jawline, while balancing the lower facial profile. | ![]() |

Post-op results are shown at 2 months. The entire procedure was done via a 2.5cm incision below the chin and it rapidly became inconspicuous. Note how much thinner the face appears after removing the submental fatty tissue and adding the small chin implant. |

Surgery
was done in the Office Surgical
Suite with local anesthesia
and mild sedation.
She returned to work the following day, and is shown here at 18 days post-op.
